A parent or guardian of the named child (if under 18) must complete this form. By signing this, you agree to give responsibility for your child to programme staff for the duration of the league and give permission for your child to play.

Hi Five Coaching may want to take and publish photographs from this event on key websites or in other media. We have a policy that prevents players shown in photographs from being directly identified, and of course we will only publish a photograph of a player if permission is given via this form.
WAIVER AND RELEASE: I do hereby waive, release, and discharge Hi Five Coaching and the specific staffs, employees, successors, and assigns of and from any and all rights and claims for damages resulting from injury to my person or property, which may be sustained or suffered by me in connection with my association with, or participating in or arising out of my traveling to or from this softball league. We, the parents or guardian, agree to the above waiver and release and we join therein. We approve of our child's attendance and certify that she is in good health and able to participate in league activities. In case of accident or illness, I give permission to the staff to acquire qualified medical treatment.
